INDIANA MEDICAL POWER OF ATTORNEY
A medical power of attorney (also known as a health care power of attorney or durable power of attorney for health care) is a legal document that authorizes someone you trust (called an agent, attorney-in-fact, or surrogate) to make medical decisions on your behalf.
INDIANA LIMITED POWER OF ATTORNEY
When a principal signs and grants a power of attorney, he/she grants either general or limited authority to the agent. The limited (also known as “specific”) power of attorney restricts the scope to the agent’s activities.
INDIANA DURABLE POWER OF ATTORNEY
A durable power of attorney simply means that the document stays in effect if you become incapacitated and unable to handle matters on your own.

A general power of attorney allow a principal to elect a legal representative, known as an “agent” or “attorney-in-fact”, to handle their financial decisions on their behalf while alive and competent.
INDIANA DEPARTMENT OF REVENUE POWER OF ATTORNEY
Taxpayers wishing to authorize a representative to have Power of Attorney on their behalf for Indiana state tax matters need to know that a properly completed POA-1 form must be submitted to the Indiana Department of Revenue: by mail with a hard-copy of the original, or by fax as a copy of the original.
